The Community Support Team provides flexible, recovery focused support for people with mental health problems living within the community.
Our aim is to provide people with a support package that enables them to feel safe and supported. This will help them move forward, set new goals, learn new skills and do things to enable them to live a fuller, more satisfying and independent life.
You will be working closely with service users, using a person-centred approach, to ensure individuals are always treated with dignity and respect.
We are looking for someone who can be flexible and responsive to the needs of service users, as directed by their individual support plans. Someone who can proactively encourage clients to identify their interests and support them in accessing social, leisure, education and work activities.
Overall, you will assist in monitoring service user’s mental health and general well-being.
