The Role

We have an exciting opportunity to become part of the team at Katherine Harriet Office Team as our Client Care Advisor,

We are looking for a highly motivated, organised and professional person to attend assessments, write care plans, take notes for meetings and be a point of call for all of our clients.

Katherine Harriet Home Care have been in the domiciliary sector for over 5 years. Established on providing high-end, quality care to enable our Clients to live independently at home. As Client Care Advisor you will work closely with the Registered Manager and as part of the Care Team to support to ensure that all paperwork and documentation leaving Katherine Harriet is done so in a timely manner, accurate and professionally completed in our house style.

Hours of work will be 08:45 – 17:15 at our Hereford Office on Broad Street.

Some of the specific duties include

  • Ensure that before any care support commences, a customer assessment and risk assessment has been completed with the Client and the care plan and supporting documentation including a clean and tidy home file is in the home environment and on the system accurately
  • Ensuring all records and reports are ready for the Directors for discussion at the weekly Monday, Care Team Meeting and minutes taken and distributed weekly.
  • Recording of all telephone conversations and emails on the computer system.
  • Ensure excellent communication with Clients, their families and representatives, staff and other health and social care professionals to deliver high quality care services and monitor.
  • Ensure that all Reviews and Telephone Reviews are up to date and recorded.
  • Maintain and ensure the Care Team maintains full and accurate records, reporting systems in accordance with legal requirements and the smooth running of the business
  • Manage quality management and improvement systems effectively managing complaints, incidents, near misses and QA’s.
  • Monitor and manage the KH calendar for the Care Team
  • To attend all Assessments / Meetings with the RM or Care Manager and take minutes and all notes on this to produce all Care Plans
  • Creation of home files with all information included.
  • Creation of Enquiry packs for prospective Clients.
  • Ensure that all Client Care Plans are tailored to the Client, and this has been created and agreed and respects the customers wishes and promotes dignity and privacy and signed off in line with our KH procedures.
  • Ensure that the Client is provided with information about the service so they are clear on what to expect and how they can raise concerns.
  • Being part of the Emergency On call rota.

Experience in the care sector preferred, starting salary £22,000.00 dependant on experience.

Due to the nature of the role a driving license will be required, use of company car will be available whilst attending appointments.
