Home First is the county wide Urgent Care and Reablement Service managed by Herefordshire Council supporting people in the community to regain maximum independence following an admission to hospital, a crisis at home, illness or disability, or any change in circumstances that has impacted their independence. Home First offer support to adults aged 18 upwards.
Our aim is to give individuals the skills, ability and knowledge to live their everyday life as independently as possible, and we do this through setting and working towards goals for a period of up to 6 week.
We want staff who choose to work with us to have control over their own work/ life balance, and therefore we actively encourage our staff to choose the hours they would like to work and their own working pattern set over a two-week rota, all we ask is that this encompasses working every other weekend.
