The Community Support Team provides flexible, recovery focused support for people with mental health problems living in with community. We aim to provide people with a support package that enables them to feel safe and supported, so that they can move forward, set new goals, learn new skills and do things to enable them to live a fuller, more satisfying and independent life. You will promote the service user’s self-esteem and enable them to express their preferences.
You will be working closely with service users, using a person-centred approach, to ensure individuals are always treated with dignity and respect. We are looking for someone who can be flexible and responsive to the needs of service users, as directed by their individual support plans.
We are looking for someone who can proactively encourage clients to identify their interests and support them in accessing social, leisure, education and work activities. Overall, you will assist in monitoring service user’s mental health and general well-being.
